Florida Homeless Education Needs Assessment

The Florida Department of Education appreciates your help in providing district-level information regarding your homeless student population. The information you provide will help us understand the needs of your students in order to better serve them.

Instructions for entering data:

  • Descriptions of each field are found below the box for data entry. If the size of the print is too small, hold down your 'CTRL' key and hit the '+' key to increase view. Hold the 'CTRL' key and hit the '-' key to reduce the view.
  • Fields marked with a red asterisk * are mandatory. You cannot submit your data if any of these fields are left blank.
  • Enter data from 2005-2006.
  • If you are unable to provide data for a certain field, enter 'NA' in the box.
  • If you cannot provide data for your entire district, please explain your sampling in the comments box provided at the bottom of this form. For example, select three schools with large concentrations of homeless students, three schools with medium concentrations, and three schools with small concentrations. It is important that your sampling represent a cross-section of your district (not just schools with high concentrations of homeless students).

Enter the number of homeless students identified in 2005-2006 for your district. If you are using a subset of schools, enter the number of homeless students represented in that sample.

If barriers exist for homeless students accessing services after-school and on non-school days, please list them here.
Number of homeless students who received transportation to school of choice on a regular basis
Average number of days that elapsed between referral made to provide transportation and actual transportation services received.
Number of homeless students who received transportation to after-school or extracurricular activities on a regular basis
If barriers exist to providing consistent transportation to school or extracurricular activities, please list them here.
Number of homeless students receiving state and federal health services
Number of homeless students receiving federal/state food services (e.g., food stamps)
Number of homeless students referred to school nurse for services
Number of referrals made to school guidance counselors for your homeless student population
